Provides a FREE community nursing program to assist with medication management. Our LPNs help with medication administration with individuals that are not able to self-manage due to various health and environmental factors. more info

Self-Management BC provides support for people living with chronic diseases such as diabetes, chronic pain, cancer, heart, kidney and lung diseases, arthritis, lupus, Parkinson, MS, etc. We offer:
- COMMUNITY WORKSHOPS (Diabetes, Chronic Pain, Chronic Conditions, Cancer: Thriving & Surviving) that are led by peer leaders and run for 2.5 hours for 6 weeks.
- HEALCH COACH PROGRAM that involves volunteer health coaches providing weekly 30 minutes telephone coaching for 3 – 6 months for individuals wanting to improve their confidence and motivation to become healthier. Individuals are paired with coaches based on gender and possibly shared chronic conditions.
