Peers Reaching Out (PRO) Program

Peers Reaching Out (PRO) is a Seniors Come Share Society pilot program funded by the Peace Arch Hospital Foundation.  PRO is designed to address the needs of older adults in the South Surrey/White Rock area, who may for health, financial or other reasons be isolated and withdrawn from their peers and community.   


PRO is a 6-week referral program that will help older adults overcome barriers blocking them from maintaining their health, independence and connection to the community. A Wellness Plan will be created with program participants focused on the older adult's existing strengths and resources.   


The program will empower them to re-engage physically and socially with the aid of a trained Peer Support Volunteer who will help participants accomplish the goals and tasks set-out their Wellness Plan.   


The Peer Support Volunteer's objective is to build a relationship with an older adult in order empower and encourage them by assisting them in following their goals that will help them achieve independence and build a supportive community. 


Peer Support Volunteers will attend ten 2-hour training sessions that will afford them the opportunity to enhance their existing abilities, as well as learn new skills strategies that will assist participants in reaching personal and program objectives. The result will be that the PRO program participant will become more active, independent, healthier and less isolated.

Referral criteria: How is one eligible for the PRO program?

Eligibility is determined by a number of factors that will be confirmed during the intake process. Candidates will be older adults that do not have a strong social network and have become disengaged and/or isolated from friends, neighbours and the community. They must reside in the South Surrey/White Rock area within the boundaries of: 196 St. to 120 St. (East to West) and 56Ave to 0 Ave (North to South). Participants will not already be actively participating in community or well-being programs and may tend to rely on the medical system to maintain their health, well-being and personal support. PRO is unable to provide support for older adults with considerable physical and/or cognitive deficiencies.
How to apply: A health care professional can fill out a referral form or anyone can call 604-531-9400 ext. 205 for self referral or if family and friends know an older adult that would benefit in the program. This form can be scanned and emailed, faxed or can be picked up by the coordinator. The coordinator will followup and setup a meeting with the older adult and perform an intake as soon as a referral is received.
